Creating a health care directive eases the decision-making process for your loved ones and ensures that your wishes are respected if you should ever face a situation where you are unable to communicate them yourself. Attending this class will help you learn about this legal written document and how to move forward in creating your own. By the end of this one session class, attendees will have the necessary forms, resources and information to be able to complete their own health care directive.

Sonja has been a hospice social worker at Aspirus St Luke's Hospice for 17 years and advocating that patient's health care wishes are followed and helping patients and families have conversations about Advance Care Planning and complete health care directives. She also provides community and staff training on Advance Care Planning for Aspirus St. Luke's Education Department
